SERVICES

Targeted Case Management-Transition Coordination
We work with Medicaid-eligible individuals who may be in long-term care facilities such as nursing homes or intermediate care facilities for people with intellectual disabilities. Our agency collaborates with the individual, their chosen support groups, and a team of specialists to explore less restrictive living options, including assisted living programs or independent living. We support individuals in navigating the complex system and simplify it to ensure their success.

Life Skills Training (LST)
Life Skills Training (LST) is a vital service within Colorado’s Home and Community‑Based Services (HCBS) program. It equips individuals with disabilities with the practical skills needed to successfully move from institutional care into community living. Through personalized instruction in areas such as meal preparation, budgeting, bill management, household organization, navigating community resources, etc., LST promotes long‑term independence and stability.
Offered through the Colorado Medicaid Waiver Program, LST plays an essential role in ensuring that individuals not only transition safely into the community but also have the tools and confidence to thrive in their chosen home environment.

Peer Mentorship
Peer Mentorship is a Medicaid‑covered HCBS service, where a person with lived experience provides guidance, modeling, and support to help another member build confidence, skills, and independence in community living.
Core Purpose of Peer Mentorship
Peer Mentors help members:
